Answering your two points:
You do need to be on-line for the auto-updates to work, of course.  I have a 
broadband connection which is on all the time, and of course, IIRC, you are 
on dial-up so that doesn't apply.  In that instance, I think the updates are 
offered when you first fire up your browser in any session, which is why IE 
popped up when you started Mozilla

If you defer updates, the list does accumulate: the way it works is that an 
entry is made in your Windows folder which identifies the updates loaded. 
Since I moved a heap of these to the recycle bin last month, I think there 
must also be a registry entry made as well, though!

Not really needing an answer, but FYI, my "run-all-the-time" stuff includes 
Windows Explorer, mail reader, and a couple of other utilities which I use 
constantly, but which I choose not to put in my start-up folder because 
there are times when I want to do a clean boot.  You may have something 
similar.
