I shoot AV almost all the time with my *istD. When shooting with handheld long 
glass, I usually switch to TV in order to makes sure I have enough shutter 
speed. I meter with the camera and check the histogram after a shot or two. I 
use matrix metering for the most part. In difficult situations I switch to 
spot. In the studio with my flash units, I shoot manual. Sometimes I take a 
flash meter reading. More often I use the remote computer control and look at 
the image and histogram on the computer screen, then adjust as necessary.

I shoot digital exclusively these days, although I may dabble in film from time 
to time.

When shooting film in my 6x7 I almost always metered with a handheld incident 
meter and set exposure manuall. With my LX, I frequently shot aperture priority 
autoexposure mode and relied on the camera meter. I would use exposure comp as 
I though necessary. With my Leica IIIF, I frequently meter with my noggin. In 
difficult situations, I may take one reading with my little VC averaging meter.
