You can easily take data that is emailed to you as an .fdf file and
import it into the original .pdf in Acrobat Pro 6, then print it.

Hi Everyone ~

I'm trying (in vain so far) to post a form on our Web site that our
clients can fill out and then return to us over the Internet. The
problem I'm having is that we don't just want the data, we want to
receive the entire form filled in with the client's information that we
can print out. One person at Adobe said he thought this was possible
with Acrobat 6.0 Professional, which we purchased, and referred me to
the pdfzone Web site for how-to advice. 

Most of the research I've done seems to point to my needing a CGI
script, which I don't know how to do. I've tried researching our ISP's
Web site, but the sample form they provide for us to tailor to our needs
results only in the data being e-mail to us. Help! My supervisor is
already telling people this service is on our Web site.
