Fixes #7369. Implement lazy parsing and caching of `application/x-www-form-urlencoded` request bodies in `MockRequest#getForms()`. Form names and values are URL-decoded as UTF-8; parameters without a value map to an empty string, and the first duplicate value is retained. Add unit and expression-generator coverage for encoded form fields, empty bodies, duplicate keys, and malformed encodings.
