Fixes #6590. Injects a `Clock` into `InstanceCheckService` while preserving the production system-clock constructor. The heartbeat test now supplies exact timestamps and no longer relies on `Thread.sleep(1)` or wall-clock resolution. Health-state checks also use one consistent time snapshot per run.
