The five new tests fail on `2.x` and pass here, and the `%d{...}{GMT+00$}`
default from the issue now expands.
Two regressions are inline on `StrSubstitutor.java`: values returned by a
nested lookup get unescaped, and `$${...}` inside an expression no longer
balances its `}`.
