1. You need to first get the server cleaned up, *then* try to get removed from blacklists. When they see you asking again next month they'll be far less likely to remove you.
2. Cleaning up the server requires determining how people got in - probably bad PHP or Perl scripts. Cleaning the server is meaningless until you close the door(s). Your question is vague, so there's little anyone can do to help. But you should probably consider whether running a dedicated server is the proper option (and to help you with that decision, here's a subtle hint: NO). You should never run your own mail server unless you're something like Google. I don't know what all happens on that server but it's likely that some outside companies provide those services and probably at a rate of no more than what's being paid for a dedicated server now. So, the "right direction" is moving to outside services and shutting that server down asap.
