I'm in Nunavut where we are obliged to queue up patiently and get raped with prejudice by the local Telco. Since Skype, I've shaved more than half off my long distance charges by using it to do personal, family and business calls to Ottawa, Montreal, Victoria, Paris, London, Brussels, Munich, Sao Paulo and other locations. Computer to computer, the sound quality often exceeds that of a local call!!! Computer to phone has given me mixed results as reported by the other users on here.

Yes,  I have and it's great when you go computer to computer.
However,  going computer to person has not been so good.  Whenever I
tried to call my friends through my powerbook/Skype, the quality of
the line was awful.  They could barely recognize anything I said, let
alone my voice.

I use it to talk to my uncle in England and we both use it through
our computers - in that case, the reception is absolutely crystal clear.

That's the only way I would use it.

A me too on the computer-to-phone! I've received calls from a Skype
user in England. It's an exercise in futility to try to have a
conversation computer-to-phone, however, computer-to-computer seemed
to work well (of course, I've only used it once).

iChat AV is another great one. The voice quality was significantly
better than what I experienced with Skype and it's fully Mac
compatible (of course, both sides of the conversation need Macs ;-).
