Joseph, This issue has occurred on this system with 15.04 and 15.10. I only "upgraded" from 15.10 to 16.04 last week, in the hopes that it might eliminate this error from occurring. It is actually a new/clean install of 16.04 due to an unresolved issue I couldn't fix, but the error has remained with all 3 versions.
Since I'm an intermediate user with Ubuntu, I will figure out how to test the latest upstream kernel to see if this resolves my issue. Also, (and please forgive my ignorance) do I add the tags to which you are referring using one of the edit buttons, or just in the comments section?
