This is in fact the way it was in Plasma 5 as well. The highlighted area is
large because you can click anywhere in that area that isn't a button to
collapse the list item.
Your confusion about what's a clickable button and what's text is
understandable, but also not the result of a recent change. Buttons being
framed vs flat is a commonly-discussed UX topic that so far we have no
consensus on. It's being discussed at
