Only the documents that are in the root are included in the PDF. If you have
documents that are in subfolders these documents are not converted into the PDF
and included in the TOC.
As a developer I want to be able to declare an item within the pdf.xml that
allows me to automatically include all the documents contained in the
sub-folders in the TOC without having to declare them by hand
