Note on the commit history: the first commit required the backing bit set to be exactly `maxDoc` bits, which also rejected a padded set whose bits past `maxDoc` are clear. That shape is correct today, so the third commit narrows it to rejecting only bits actually set at or beyond `maxDoc`, and restores the `applyMask` javadoc and the `testApplyMaskThrowsOnBitsBeyondLiveDocs` case that the first one removed.
