`MonotonicBlockPackedReader` and `MonotonicBlockPackedWriter` are used in
the `FixedGapTermsIndexReader` and `FixedGapTermsIndexWriter` respectively.
Those readers/ writers from internally from PackedInts#getReader. This PR
removes that dependency by using internally DirectReaders and DirectWriters
instead.
