My recollection is that Ingo was convinced to make the change, but
either hasn't yet, or hasn't done a mandoc(1) release since he did.

Sorry I don't have citations for either of these claims.  My
web-search-fu failed me.

I do recall that it was Ingo who brought to my attention that full
("shouting") capitals are deprecated on the Web for accessibility
reasons because (some) screen readers will spell the words out
letter-by-letter that way, which would be a mightily tedious thing to
endure with man pages.
