I've installed 4.2.99 extras and tdfx driver from directory bleeding edge:

resolution modes and/or a smaller virtual screen size

My Vooodo3 has 16MB of video memory. I've decreased resoulution to 640x480, but
it didn't help. Everything works well on 4.2.0 and binary driver from 20021022.

Full log attached.
From your log:
(--) TDFX(0): Virtual size is 1920x1440 (pitch 1920)

There is a small bug that appeared on the list already some time ago, where the 2D-code thinks it's using high virtual resolutions.
Back then David Dawes wrote in
http://sourceforge.net/mailarchive/message.php?msg_id=2328996 :

"It's a side-effect of the recent RandR additions. It should be
resolved before too long. In the meantime, you can specify the
virtual size explicitly."

Could you try if that helps?
