I suspect that these prompts for PKCS #11 provider
passwords are caused by the new smart card monitoring
thread in Firefox and Thunderbird 1.5.

The smart card monitoring thread monitors smart card
insertion and removal events.  It prompts you for the
smart card's password when it detects that the smart
card has been inserted.  The intention is so that it
can switch the application to a "secure" mode of
operation upon card insertion, with extra security
features enabled.

It is possible that the smart card monitoring thread
doesn't handle an already inserted card very well.
