That shows three monitors connected and active. Was the problem occuring
at the time you ran the command?

Please also run this command while the problem is happening so we can
see what the compositor thinks the current configuration is:

  gdctl show > gdctl.txt

and attach the resulting text file here.

It may also pay to try Nvidia 575 using the Additional Drivers app.

Title:
[nvidia] One monitor doesn't show any output after boot

Status in mutter package in Ubuntu:
  Incomplete

Bug description:
One of my monitors does not show the screen when Ubuntu boots, like
the other monitors do, it just hangs on a black screen like it is not
receiving any input. The only workarounds I have found so far are:

  1. Unplug and re-plug the monitor
  2. Turn the monitor off in the display settings, apply and keep changes, then
turn it back on

This did not exist on 24.04 or 24.10, but I have reproduced in 25.04
and 25.10. The monitor is an "Acer Nitro 34" 21:9 Curved UWQHD", and
it works fine on Windows 11 as well, the problem appears to be
  exclusive to the newer Ubuntu.
