My Pinnacle (card=39) shows NTSC for the video mode: "tuner 2-0061: type set to 2 (Philips NTSC (FI1236,FM1236 and compatibles)" Your line shows "PAL+SECAM" which probably is causing the... "Fatal Error: PLL's didn't lock."
