I set it up as per yours above, and it works like a dam. Wiping out gpm helped, I think. One weird (?) thing I noticed that probably caused much of the problem was the presence of two mice in XF86Config-4: a generic and the Logitech. Both were listed as live. Naturally, I have it set for one only.
