Thanks for the information, this is finaly a clue what is going on.

Is there a pvscan or similar task running?  This is the only time where
the state info is set this way.

Yeah.  pvscan should be run by udev for each new device.  For some
reason this either don't work, breaks in the middle or no idea what
happens.
