Okay, this was bogus -- I had previously installed otrs and the otrs
mysql database user was not removed when I uninstalled it. When it went
to create a database with a different password for the same user, that
caused the error. Completely removing the mysql user 'otrs' or changing
to the original password both fix the problem.
