Both: the key words are “no empty line”, since po4a uses empty lines to
split paragraphs, if I understood correctly. Maybe another “before”
could make the sentence a little bit more explicit.
Anyway, I guess a simple test can illustrate the issue, see the attached
file (and how broken it is in the first place when opened with perldoc
anyway, so maybe the end of the sentence should simply be dropped), and
the resulted PO file:
po4a-gettextize -f pod -m foo.pod -p foo.pot

The sense is not the same (it's even the contrary), and I don't think
your proposal is accurate.
