I had sysv-rc, sysvinit, sysvinit-utils, and initscripts 2.86.ds1-61 installed. Then I installed the same packages, version 2.88dsf-7. This had the effect of installing insserv version 1.14.0-2. Then I saw this error in the syslog: init: Id "z6" respawning too fast: disabled for 5 minutes (This is the z6 line in /etc/inittab: z6:6:respawn:/sbin/sulogin). I tried to execute a shutdown, but the shutdown command had no effect. I reverted to sysv-rc, sysvinit, sysvinit-utils, and initscripts 2.86.ds1-61. I had to reboot using the hosting company's web interface. But after I rebooted, the respawning error message disappeared, and I could shutdown normally. To test that the previous odd behaviour was associated with insserv, I then reinstalled version 2.88dsf-7 of the sysv and initscripts packages, along with insserv 1.14.0-2. Then I rebooted. Thereafter I saw exactly the same behaviour (respawning message in syslog; failure to reboot) that I'd seen before. I reverted to 2.86.ds1-61 of the sysv and initscripts packages (which removed insserv) and the problems disappeared again.
