Before Raphaƫl showed me this report, I had written my own.
I include it here simply because it goes a step further than the
quilt format and advocates aborting in the presence of any changes
outside debian/*:

  An option like --refuse-debian-patch (v3 mainly) or
  --refuse-non-debian-changes (more generic) would be useful, which
  would abort creation of a source package if the diff containned
  changes outside of debian/*.

  Ideally this could be enforced with a file in debian/source/*.

  While the automatic generation of
  debian/patches/debian-changes-$version is useful at times, some
  packages specifically want to adopt a patches-for-everything
  policy, and such an option would help enforce that.

  (I have uploaded packages in the past without noticing the
  auto-generated debian/patches/debian-changes-$version file).
