I'm just about to release Xpdf 3.01 which will have code to handle that
correctly."

Sometimes xpdf bugs go away just by recompiling xpdf with new freetype
versions. Have you tried recompiling xpdf with freetype 2.1.10?
(I haven't and can't try it until later today.)
