I still ended up with the sound muted. It appears that
alsactl restore
wasn't being run as when I ran it after kde started, the un-muted sound
levels were restored.
It appears that the alsa-utils start script is only run from /etc/rcS.d/
- is this correct:
ls -al /etc/rc*d/|grep alsa
