That's gtk-missing-image, the first one (popup-blocker.png) is the 24x24 image downscaled, and the other one (rss-feed.png) is the 16x16 variant. I'm not sure why it chooses different sizes for them, or if it's relevant to the bug. Anyway, the fact that epiphany refuses to load the popup blocker icon means that this isn't a bug specific to the extensions.
