When the smarthost+fetchmail configuration preset is selected in debconf, the config should automatically include smtp_accept_queue_per_connection = 0. It is typical for smarthost+fetchmail configurations that mails come in in batches from a fetchmail run when the system gets online. These batches are very often larger than the smtp_accept_queue_per_connection default of 10. Thus in the default config, mail from fetchmail is unnecessarily delayed. I don't think that in a typical smarthost+fetchmail leaf setup, there will be a workload that needs smtp_accept_queue_per_connection > 0 for load limiting.
