   * What led up to the situation?

     I was trying to repair an exfat volume (USB stick) that had not
     been cleanly unmounted.

   * What exactly did you do (or not do) that was effective (or
     ineffective)?

     I ran `fsck /dev/sdXn`

   * What was the outcome of this action?

     It apparently didn't complete but rather output the following:
     ERROR: illegal UTF-16 sequence.
     BUG: failed to convert name to UTF-8.

     In case it's helpful, here's a full transcript of running this
     through gdb:
