I can actually reproduce with the following: \documentclass{article} \begin{document} \tableofcontents \end{document} NB. no images, no graphicx, etc. This makes the PDF (again, modulo {Creation,Mod}Date headers) reproducible within the same second, which suggests a time-based random seed generation.
