Actually, it make sense in my case, as it make it a lot easier to
check the original paper book when I find errors in the docbook text,
and also make it easier to insert indexterm entries into the correct
location and with the correct range.

Sorry to hear you do not want to apply the simple patch, but I can
live with the workaround.
