So it would be easier to just forward port 5060 on both sides and use SIP?

I'd guess this could be done with some creative signalling using  'custom 
headers' in iax2. i don't think custom headers are supported  by default, 
though, but if not, there's a patch  out there (IIRC at  bugs.digium.com)
