I'm using the most recent version of `texinfo.tex`; my `test.fn` looks
the same as yours, so this isn't an issue. It is rather a problem
with the `texindex.awk` script.
Attached is the version of `texindex.awk` that I get after compiling
current git with
./autogen.sh && ./configure && make -j12 && sudo make install
My `awk` version on my openSUSE 16.0 Leap box identifies itself as
follows.
GNU Awk 5.3.2, API 4.0, PMA Avon 8-g1, (GNU MPFR 4.2.1, GNU MP 6.3.0)
I directly called the awk script with
/usr/bin/awk -f texindex.awk test.fn
