My solution for this type of problems has been Default Folder 
(https://www.stclairsoft.com/DefaultFolderX/) for many, many years, almost as 
many as I have been using BBEdit (Light, TextWrangler,...), from the pre OS X 
era.

Yes, Default Folder is paid but it is sooooo much more convenient, and the 
learning curve is almost nothing. I always wondered why Apple hasn't done 
something similar.

By the way, after many years of using BBEdit for free, last week I took the 
plunge and paid the license!
