Go to http://www.bmc.com/support/licensing-passwords/ click on 'Request a New License' Sign in, and agree to the terms click on the 'View Current Licenses' tab.
This should provide you with a list of every license your contract has ever had, it's current state, PO, etc...including the Host ID that the license was issued against.
