I am not sure how you couldn't but did you successfully load Atrium CMDB7.6.4? Did you look at the log files after the installation? Did you run the verification check it offers to run? Simply loading the form will not necessarily fix this issue. During the install records are created with the names of the forms and if those are not there it will not see the Bulk form, an example would be the 'Schema Names' form.
