It's a generic document about customizing notifications in ITSM. The specific answer you are looking for (fields on approval related forms), I suspect, is not there, but it's a good reference document and might give you some leads. I was a self-described ITSM 7.x notification expert, but I haven't touched it in a while and I can't find my old notes at the moment. If I locate anything useful, I will send it your way. Worst case...turn on server side filter logging and trace the filters to see which one you need to update. Tedious, yes, but it's your ultimate and surefire weapon. Regards.
