Can you connect using SQL Server management studio? Just for grins, try inputting port 1033 instead of 1433 in the database setup section of the ITSM install.

I did the below changes and restarted the server, but no luck. And along with that I did the changes in the SQL Surface configuration, where in I opted TCP/IP and Name option, even it did not helped me out in this.
