A long long long time ago a client of mine once had this issue.. I had gone under the covers, and realized that it the ARS was trying to create this object with the same ID that already existed (I think it was for AL's) but the ID in the container table was not updated. We never were able to find out how that may have happened but we attributed it to possible database timeouts as they were experiencing a few of those during that time..
On another occassion a lister here had experienced the same thing and I was able to help him/her restore their system.. The thing about such errors is usually it doesn't give you details as to which table or container this error is resulting from, and one way to find out is an SQL log during the operation. I'm not sure there could be another method to find this out as this is a DB problem..
