Actually, that wouldn't be how it worked, but if he's using ITSM 7, it's possible that Incidents are being discarded (i.e. opened but not saved by the user). Those are allocated an Incident ID when the New window is opened.
If he's on ITSM 7, he can go to the HPD:IncidentInterface_Create form - it will show the records in that "pre-Incident" state.
