Hi, Herb:
Strip off the Modify mode parameter. It is not
necessary when using the eid parameter. Exclude the quotes and equal sign
from the eid parameter.
One more thing. For me, in the 5.1.2 version of
mid-tier the formalias reference rather than form seemed to work.
