I've recently got one or two of them over the last month or so as well. They were related to instrumentation which I don't have set up for the app. In the one case where the user gave me anything else useful, they said they were getting it on an Evo 4G.
I suspect that there is a bug in someones OS deployment or a custom ROM thats out in the wild because I was not able to reproduce it or even find a place (that I could control) where it might be happening.
