You might want to run with the following added to release_object just
before the "assert( obj->refcount );" to get enough information to be
able to debug the issue if it happens again:
if (!obj->refcount)
obj->ops->dump( ptr, 1 );
Also, you might want to change the assert into "*(char *)0 = 0" and
enable coredumps (ulimit -c unlimited) so that a core file is
generated that you can debug further with gdb, if necessary.
