After further investigation.. It appears the form fields are stored as annotations.

PDDocumentCatalog docCatalog = document.getDocumentCatalog(); PDAcroForm acroFom = docCatalog.getAcroForm(); List<PDField> fields = acroForm.getFields(); But when I create the document using pdf box it is able to recognise the form elements.
