I do not understand why this is so complicated. You dont see Microsoft or apple or google asking you to log in to a web page and diddle with some kind of ticket system... just ask for permission and collect the info. If you need to probe more and dig deeper, fine... go a head... Ask if you may contact the user... why all this?
