1) To clarify, did this issue start after an update? If so, which and
when precisely?

2) Please provide the ouput of the following terminal command (not manually 
attach files):
ls -la /var/crash

3) To verify a WORKAROUND, if you install gnome-session-flashback does
this crash also?

4) To advise, keep in mind that this issue may be due to upgrade cruft
being left behind from your OS upgrade. Hence, testing
http://cdimage.ubuntu.com/daily-live/current/ would help rule this out.
