I also suffer this problem from 14.04 onwards. Happens occasionally (maybe once a day on average) and typically after a reboot... The keyboard setting from the Language Icon in the top right is set to 'English UK' - there is also an 'English US' option - and simply re-selecting English UK fixes it.. The next time I reboot it's a 50/50 whether it stays on UK or reverts back to US.
