This looks like it may be an X bug.
/etc/default/console-setup ends up with the correct options:
XKBMODEL="pc105"
XKBLAYOUT="us"
XKBVARIANT="dvorak"
XKBOPTIONS="lv3:ralt_switch"
Which X gets:
