None of the previous suggestions worked for me. After investigation, I found that my vga setting in /boot/grub/menu.lst (0x031b, which is 1280x1024) did not match the resolution in /etc/usplash.conf (1024x768). Changing /etc/usplash.conf to 1280x1024 resolved the issue. I expect that changing /boot/grub/menu.lst to 1024x768 would have worked as well.
