Can this be somewhat similar to <a href="https://bugs.launchpad.net/ubuntu/+source/usplash/+bug/140892";>bug #140892</a>? Try changing 'splash' to 'nosplash' in your /boot/grub/menu.1st file. Also, if you remove the 'quiet' option you would at least see whats going on when it fails.
